Abstract

Prostate adenocarcinoma is one of the most common malignant tumors affecting males worldwide. Histopathological grading, particularly the Gleason scoring system and International Society of Urological Pathology (ISUP) Grade Group classification, plays an essential role in evaluating tumor aggressiveness, prognosis, and treatment planning. Prostatic adenocarcinoma is graded by the Gleason system, based on glandular architecture at low magnification. The system assigns patterns 1 to 5, with pattern 1 most differentiated and pattern 5 undifferentiated, and sums the primary and secondary patterns into a score from 2 to 10. The 2014 ISUP Grade Group classification maps this to five groups: Group 1 [3+3=6], Group 2 [3+4=7], Group 3 [4+3=7], Group 4 [4+4=8], Group 5 [9-10]. Grade is a key predictor of behavior and treatment decisions.  Transrectal ultrasound-guided 16-core prostate biopsy series and contemporary literature evaluating distribution of Gleason scores and Grade Groups
